This website is operated by Shafi Lodhi, MD. It describes a forensic expert witness practice and accepts case inquiries from attorneys and other retaining parties. Questions about this notice can be sent to forensic@shafilodhimd.com.
What the site collects
The case inquiry form collects what you choose to enter: your name, firm, email address, and telephone number, and details of the matter such as case type, jurisdiction, dates, the retaining party, approximate records volume, the parties and counsel, any opposing experts, and a short case summary. Email and telephone inquiries contain whatever you send.
The site also uses Google Analytics, which sets cookies to record the pages visited, approximate location at the city level, device and browser type, and how a visitor arrived at the site. This information is reported in aggregate and is not used to identify individual visitors.
How inquiry information is used
Inquiry information is used to respond to you, to check for conflicts of interest, and to decide whether a matter fits Dr. Lodhi's practice. It is not sold and is not used for advertising.
Where it is stored
The site is hosted by Netlify, which receives and stores form submissions. Each submission is also sent by email to forensic@shafilodhimd.com, an account hosted on Google Workspace.
Confidential and medical information
Please do not send medical records, other protected health information, or privileged material through the form or by email before retention. This site is not used for patient care, and nothing submitted through it becomes part of a medical record. Patients seeking care should see where Dr. Lodhi practices.
How long it is kept
Inquiry information is kept as long as needed to respond and to maintain records of conflict checks. You may ask for an inquiry to be deleted by writing to forensic@shafilodhimd.com; information needed to document a conflict check may be retained.
Analytics choices
You can block or delete cookies in your browser settings, or install Google's Analytics opt-out browser add-on.
Changes
If this notice changes, the effective date above will change with it.
